凡擔任教學型兼任助理實際參與協助課程滿1學年，並履行教學型兼任助理相關規定及繳交成果紀錄，同時通過教學型兼任助理期末考核者，可提出服務證書申請。

Those who have been TA/ATA and participated in assistant courses for one academic year, and follow the Enforcement Rules of Teaching Assistant  and submitted the performance records, at the same time, passed the final assessment of TA/ATA can submit an application for a service certificate.

  • 認證審核作業原則
  • Before you apply for TA and ATA Certificate you must meet the following requirements
    1. 參與基礎培訓課程 
      must attend the TA/ATA basic training course
    2. 擔任學期參與至少1場次知能成長活動 (兼任助教需要至少2場次) (請至學生資訊系統「D.M.11.教育訓練個人紀錄」查詢已参加場次)
      must attend the TA/ATA advanced training course once (ATA need to attend twice)

    3. 至少協助1學年之課程
      must been TA/ATA and participated in assistant courses for one academic year
    4. 學期末於期限內完成繳交教學成果紀錄 
      submitted the performance records before deadline
    5. 教學型兼任助理考核平均值高於3.5分(含)以上  
      The average value of assessment of TA/ATA higher than 3.5
  • 申請表：

    高雄醫學大學為協助學生申請教學型兼任助理服務證書之目的，須蒐集您的姓名、電話、學號等個人資料(辨識類：C001辨識個人者)，作為證明參與活動及台灣地區內進行聯繫之用，個人資料將保存1個月（將依實際狀況調整）。本校於蒐集您的個人資料時，如有欄位未填寫，將可能對您的活動聯絡及證明申請等後續作業流程有所影響。如欲行使其他個人資料保護法第3條的當事人權利，請洽本校教務處教學發展與資源中心。

    For the purpose of assisting teachers to recruit TA or ATA, Kaohsiung Medical University must collect personal information such as your name, phone number, job number (identification category: C001 individual identification), as proof of participation in activities and contact within Taiwan. Personal information will be used Save for 1 month (will be adjusted according to the actual situation). When our school collects your personal information, if some fields are not filled in, it may affect your follow-up procedures such as activity contact and certification application.
